Endocrinology Patients UConn Health

Endocrinology is the study of the endocrine system. Endocrine glands secrete hormones into the bloodstream that regulate the function of other cells and organ systems of the body. The endocrine system consists of separate glands such as the adrenals, thyroid, parathyroids, pituitary and pancreas. Common endocrine disorders include osteoporosis, diabetes and hypothyroidism.

An endocrinologist is a physician who studies and treats imbalances of the endocrine system. Endocrinologists play important roles in patient care and academic research. They are experts in their field and should serve as your main physician for endocrine disease-related problems.

The thyroid gland, located in the neck, makes and secretes thyroid hormone which affects nearly all tissues of the body. Its function is to regulate the bodys metabolism. Common thyroid problems include:

Hyperthyroidism the production of too much thyroid hormone. It can produce symptoms such as palpitations, sweating and weight loss.

Hypothyroidism the production of too little thyroid hormone. It can produce symptoms such as fatigue, weight gain and constipation.

Thyroiditis inflammation within the thyroid gland.

Goiters enlargements of the thyroid gland.

Solitary thyroid nodules growths within the thyroid gland.

Thyroid cancer a malignant growth within the thyroid gland.

The parathyroid glands are four small glands located behind the thyroid gland which secrete parathyroid hormone, controlling the bodys calcium levels.

Hyperparathyroidism a disease of calcium balance is one of the most common disorders of the parathyroid glands. The pancreas is located behind the stomach and secretes insulin, glucagon, somatostatin and other hormones. It controls blood sugar and glucose metabolism as well as helping control other endocrine cells of the digestive tract.

Diabetes is the most common disorder of the pancreas.

The adrenal glands are located on top of the kidneys and secrete adrenaline, aldosterone and cortisol. These hormones maintain salt balance, blood pressure, and help maintain fluid concentrations in the body. Common adrenal disorders include Addisons Disease, Cushings Syndrome, hyperaldosteronism and adrenal tumors.

The pituitary gland is located at the base of the brain and secretes hormones which control the activity of other glands such as the ovaries, testes, adrenals and thyroid. Common pituitary disorders include pituitary tumors, such as prolactinomas and hypopituitarism, a disorder in which pituitary hormone secretion is abnormally low.

About 27 million Americans live with Type 1 or Type 2 diabetes. Diabetes is the leading cause of kidney failure, non-injury lower-limb amputations and new cases of adult blindness in the United States. Hypoglycemia, or insulin shock, occurs when blood sugar is too low, and can be very dangerous. At UofL Physicians-Endocrinology, our goals are to provide outstanding clinical care for patients with Type 1 and Type 2 diabetes mellitus; to educate patients, the public and members of the health care profession about diabetes; and to advance understanding of diabetes by conducting high-quality research.

We work closely with primary care providers and with other UofL Physicians who are skilled in the diagnosis and treatment of diabetic complications, to create a large, multidisciplinary network to provide optimal care. Our diabetes program includes training in carbohydrate counting, insulin self-management, and insulin pumps. Our physicians and certified diabetes educator (CDE) provide education and support on how to manage diabetes and prevent diabetes complications.

Low HDL (high-density lipoprotein) and high LDL (low-density lipoprotein) are associated with heart disease and stroke. Our practice can help you lower your LDL cholesterol, raise your HDL cholesterol and reduce triglycerides while minimizing adverse side effects.

The pituitary gland is called the master gland because it controls the complex function of most other endocrine glands and is located at the base of the brain. We offer a multidisciplinary approach to treating patients with pituitary and hypothalamic disorders including acromegaly, Cushings syndrome, prolactinoma, chromophobe or non-functioning adenomas and thyroptroph adenomas, and the many conditions that cause hypopituitarism, as well as diabetes insipidus and low sodium (SIADH).

The adrenal glands produce hormones responsible for the maintenance of metabolic processes, salt/ water regulation and the response to stress. These hormones are cortisol, aldosterone and epinephrine. Addisons disease, hypertension with low potassium and pheochromocytoma are among the endocrine disorders of the adrenal glands that we treat.

Our Metabolic Bone Disease Program provides care for patients with primary hyperparathyroidism, hypoparathyroidism, Pagets disease, renal bone disease, disorders of vitamin D metabolism, male osteoporosis and transplantation-associated bone disease. We offer bone density testing for the diagnosis and follow-up of osteoporosis and other metabolic bone diseases. We work closely with our UofL Physicians endocrine surgeons to provide minimally invasive surgical treatment for parathyroid tumors. We also offer bone density testing for the diagnosis and follow-up of osteoporosis and other metabolic bone diseases.

We provide comprehensive medical services for patients with reproductive endocrine problems, including menstrual cycle abnormalities, polycystic ovarian syndrome (PCOS), menopause, premature ovarian failure, male hypogonadism including late onset male hypogonadism and pubertal disorders.

Patients with thyroid disorders include those with hyper- and hypothyroidism, thyroid nodules and thyroid cancer. Hyperthyroidism occurs when the body produces too much thyroid hormone causing symptoms including nervousness, heat intolerance and weight loss. In hypothyroidism, by contrast, the body produces too little thyroid hormone causing fatigue, cold intolerance, hair loss and weight gain, among other symptoms. At least 10 percent of women have hypothyroidism by the age of 50. We offer ultrasound-guided fine needle aspiration of thyroid nodules including molecular testing, and we work with our Nuclear Medicine colleagues to use radioactive iodine (RAI) to scan or destroy thyroid cells that are overactive or malignant.

Our endocrinologists and nurse practitioner are highly trained specialists in endocrinology services and metabolic disorders in the Louisville area. They have received training at some of the finest institutions in the country, including the National Institutes of Health, Albert Einstein Medical Center and Emory University. They hold leadership positions in national organizations, including the Endocrine Society and the American Diabetes Association, and serve as consultants on NIH panels and on Veterans Affairs (VA) study sections.

As part of the UofL Physicians team, our endocrinologists frequently meet with other UofL Physicians specialists to evaluate and discuss our patients treatment. These specialists include ophthalmologists, nephrologists, radiologists and nuclear medicine specialists.

UofL Physicians-Endocrinology also maintains inpatient consultation services at Jewish Hospital, Norton Hospital, the Louisville Veterans Affairs Medical Center and University of Louisville Hospital. We do more than 200 initial inpatient consultations per year.

Welcome to the website of the Section of Diabetes and Endocrinology at the University of Oklahoma Health Sciences Center. This site will introduce you to our doctors, researchers, and staff, and summarizes our clinical, educational, research, and diabetes prevention programs. Our Section is a core component of the Harold Hamm Diabetes Center at The University of Oklahoma.

Our clinical work encompasses the full spectrum of Diabetes and Endocrinology, including thyroid, adrenal, pituitary, bone, gonadal, and metabolic disorders. Outpatients are seen by physician referral, and have the opportunity to participate in clinical trials. Diabetes services are undergoing rapid expansion in collaboration with many other disciplines and partners on and off campus. Our goal is a state-wide effort to improve the lives of all people affected by diabetes and other endocrine disorders.

Our professional educational programs are aimed not only at medical students, residents, and fellows, but also at the broad spectrum of health care providers in hospitals and in the community who must work as a team to provide first class clinical care. We also place a major emphasis on education of communities, patients, their families, the general public, and makers of policies that affects community health.

Our clinical fellowship program, with six RRC-approved positions, seeks academically-oriented physicians for 2-, 3- and 4-year training programs. There are opportunities for experience of clinical and basic science research during the fellowship.

Our research program is a cornerstone of the Section. Its major emphasis is on diabetes and its vascular complications (eye disease, kidney disease, neuropathy, and accelerated atherosclerosis (hardening of the arteries)). We conduct clinical and basic science research with strong emphases on collaborations between clinicians and basic scientists, on linking different disciplines that are relevant to diabetes, on building partnerships with communities, particularly minority communities that are severely afflicted by diabetes, and on involving trainees in research. Our program has brought more than 40 new extra-mural grants and many new investigators to our Section in the past four years. Two major, five year NIH program grants were awarded in late 2007: a Diabetes Center of Biologic Research Excellence, and in collaboration with the OU College of Public Health, an Oklahoma Center on American Indian Diabetes Health Disparities. In 2012 the CoBRE "Diabetes Center of Biologic Research Excellence" was awarded a five year renewal under the leadership of Dr. Jian-Xing Ma.

The Division of Endocrinology has a 50-year tradition of excellence in basic and clinical research, education and patient care.

Interdisciplinary research programs have been emphasized and links forged to the Pediatrics research program and the diabetes and osteoporosis programs at the Graduate School of Public Health. The fellowship program is supported by a National Institutes of Health (NIH) training grant.

Excellent clinical care is provided on inpatient consult services at the Veterans Affairs Pittsburgh Health Care System (VAPHCS), Presbyterian/Shadyside locations of the UPMC Health System, Magee Womens Hospital, and the outpatient Falk Endocrinology Clinic on the UPMC Presbyterian campus. Many endocrine faculty are listed among the "Best Physicians in the U.S." in the care and management of diabetes, obesity, thyroid disease, adrenal disease, pituitary disease, osteoporosis, and other Endocrine-related illnesses.

Researchers at the Division of Endocrinology and Metabolism, Pitt School of Medicine, have identified a cell-signaling pathway that plays a key role in increasing insulin secretion during pregnancy and, when blocked, leads to the development of gestational diabetes. Endocrinology Clinic at Parnassus | UCSF Medical Center

Parking at Parnassus

Public parking for an hourly fee at UCSF Medical Center is available in the seven-level Millberry Union Garage at 500 Parnassus Ave. There are two garage entrances one on the north side of Parnassus Avenue and another on Irving Street, just east of Third Avenue.

Another garage with an hourly fee, at 350 Parnassus Ave., is open Monday to Friday from 6 a.m. to 10 p.m.

Metered street parking is rarely available.

Patients being admitted to the hospital may be dropped off at the circular driveway leading to the main entrance at 505 Parnassus Ave. This area also may be used to pick up patients who are being discharged.

For more information about parking at Parnassus, call Campus Parking Services at 476-2566.

Valet parking service is available at the Ambulatory Care Center (ACC) at 400 Parnassus Ave. from 8 a.m. to 3 p.m. The valet service is free but patients must pay regular parking fees. For more information about the valet service, call (415) 476-6200.

A UCSF "greeter" also is available at the ACC from 8 a.m. to 5 p.m. to assist patients find their way.

UCSF Medical Center is accessible via Muni streetcar line N-Judah*, which stops at Second Avenue and Irving Street, and the following Muni bus lines, which stop in front of the hospital:

In one weekend, the diagnosis was made and surgery targeting the specific genetics of the patients condition was conducted. In the past, endocrinologists would have been limited to testing that only sequenced a handful of genes.

Cleveland Clinic Florida is South Floridas premier medical center for the treatment hyperparathyroidism, parathyroid tumors, parathyroid disease and parathyroid hormone problems. Our staff of expert surgeons performs routine minimally invasive parathyroid surgery as an outpatient procedure to treat patients in Broward County with single gland adenoma, the most common cause of hyperparathyroidism.

Single gland adenoma accounts for about 99% of hyperparathyroidism cases and occurs when one of the four parathyroid glands is enlarged and produces too much parathyroid hormone (PTH). PTH regulates calcium levels in the blood. Excessive PTH secretion may lead to hypercalcemia or elevated calcium levels, which may lead to bone density complications.

Using minimally invasive surgery, our surgeons have achieved a success rate of over 95% in curing single gland adenoma hyperparathyroidism.

Non-invasive preoperative imaging tests help surgeons to locate the affected gland. This may include ultrasound or a technique called Sestamibi scan, which uses a chemical to locate the enlarged parathyroid gland. Once the enlarged gland is determined, the surgeon then makes a very small cut, about 1 inch, in the neck. Typically, the cut is made in an inconspicuous area or within a crease. A small probe is inserted into the cut and the diseased gland is removed. The entire procedure is completed in less than 30 minutes.

There is a minimal amount of pain associated with the procedure. Pain medication will be prescribed; however, many patients report that they do not need it. Following the procedure, patients can go home; no overnight hospital stay is necessary. Many patients are back to work within a week.

Occasionally, conventional parathyroid surgery is required if the surgeon cannot determine which gland is affected using preoperative imaging techniques. Conventional parathyroid surgery can also be an outpatient procedure, but it sometimes requires an overnight stay. During a conventional surgery, the surgeon will surgically evaluate all four glands to find the one that is enlarged.

A unique multidisciplinary integration of medical and surgical treatments ensures patients receive continuity of care under the guidance of internationally renowned physicians and surgeons.

Patients follow up with their endocrinologist whether they undergo a minimally invasive or conventional parathyroid surgery to treat their single gland adenoma hyperparathyroidism. The endocrinologist will monitor the patients parathyroid hormone and calcium levels. The endocrinologist may also prescribe calcium supplements to rebuild bone density loss.

There are other types of hyperparathyroidism for which surgery is not an option and other forms of treatment may be recommended. The surgeon will determine whether a surgical approach is the best option based on the patients individual disease.

The Diabetes Center

Within the Endocrine Division, the Diabetes Center, established in 1997, provides the highest standard of care for approximately 2,000 patients with both type I and type II diabetes mellitus. It offers a multidisciplinary team approach for diabetes care with comprehensive case management and development of an individualized treatment plan. Our expert team is composed of diabetologists, certified diabetes educators and registered dieticians who are experienced in intensive diabetes management, including insulin pump therapy. Community Functions

Our director, Dr. Dandonas pioneering work in the diabetic center lead to his appointment as an advisor to Independent Health, Blue Cross, Blue Shield and to Community Blue in matters related to diabetes. Weve authored algorithms in diabetic management, which we disseminated to primary care physicians so that care of diabetes can be improved and its complications prevented.

Various community programs have been set up by the Center for the city, which include: Public health surveys which screens for hypercholesterolemia, diabetes, hypothyroidism and hypertension starting in 1995 and then annually thereafter. Data from initial surveys have shown us for the first time that 2/3 of Buffalos population has hypercholesterolemia (high cholesterol) and that the prevalence of diabetes in the region is eight percent. So far 9500 Western New Yorkers have been surveyed. Every year we do our best to raise funds for this activity which amounts to $45,000-50,000. These screenings are done in association with the Erie County Health Dept.

A unique Diabetes Patients Forum which meets every two months, since October 1996, which aims to empower and educate diabetic patients and their families. This forum aims to educate patients and their families regarding their conditions and empower them to ask questions regarding the management of their disease.

May: 2013

American College of Endocrinology and American Association of Clinical Endocrinologists, Phoenix, AZ This meeting was held in early May and one of the 10 chosen oral presentations was the one on the beneficial effects of testosterone in patients with hypogonadotropic hypogonadism. This led to a press release. Several abstracts based on clinical cases were presented by the fellows. These unique cases also raised a lot of interest.

Dr. Dandona participated in global consultants meeting related to the new anti-diabetic drug, TAK-875, from Takeda Pharmaceuticals in London, UK. This new class of drugs binds to a fatty acid receptor on the -cell in the pancreatic islet and induces the release of insulin. The fall in HbA1c is just under 1% and there is no risk of hypoglycemia. Dr Prabhakar Viswanathan, Senior Medical Director in Takeda, is leading the program to develop this drug. Dr Viswanathan is an ex-fellow from our research program from UB. He is the third fellow trained by Dr Dandona to hold such a senior position in a major pharmaceutical company. Drs Davinder (Dave) Gill and Jaswinder (Jaz) Gill (Sanofi-Aventis) are the other two.

Department of Medicine Research Day This day witnessed an outstanding performance by our division with the prizes for the best research and best case presentation being awarded to Manav Batra and Cherie Vaz.

Immunometabolism Symposium, Toronto Dr Dandona attended this meeting and contributed to the discussions during this meeting in relation to our groups large body of work linking metabolic states and macronutrient intake to oxidative stress and inflammation.
