Cellular & Gene Therapy Products

The Center for Biologics Evaluation and Research (CBER) regulates cellular therapy products, human gene therapy products, and certain devices related to cell and gene therapy. CBER uses both the Public Health Service Act and the Federal Food Drug and Cosmetic Act as enabling statutes for oversight.

Cellular therapy products include cellular immunotherapies, and other types of both autologous and allogeneic cells for certain therapeutic indications, including adult and embryonic stem cells. Human gene therapy refers to products that introduce genetic material into a persons DNA to replace faulty or missing genetic material, thus treating a disease or abnormal medical condition.

Although some cellular therapy products have been approved, CBER has not yet approved any human gene therapy product for sale. However, the amount of cellular and gene therapy-related research and development occurring in the United States continues to grow at a fast rate. CBER has received many requests from medical researchers and manufacturers to study cellular and gene therapies and to develop cellular and gene therapy products. In addition to regulatory oversight of clinical studies, CBER provides proactive scientific and regulatory advice to medical researchers and manufacturers in the area of novel product development.

Multiple Sclerosis

At present there are no treatments that specifically target the abnormal immune responses in MS. Current approaches, such as interferon, copaxone, or immune suppressants all act in a nonspecific manner blocking immune responses against the myelin sheath. While these approaches are useful for reducing the severity of disease, they do not repair the damage to nervous system tissue that has already occurred and therefore they cannot cure multiple sclerosis.

Mesenchymal stem cells (MSCs), have immune regulatory properties which may stop the immune system from attacking the myelin sheath.

Mesenchymal stem cells may also potentially help remyelination (re-generation of the myelin sheath) of the affected neurons. Currently, the University of Cambridge is conducting formal clinical studies using mesenchymal stem cells for treatment of MS.

The adult stem cells used to treat MS at the Stem Cell Institute are called allogeneic mesenchymal stem cells. They are harvested from human umbilical cords donated after normal, healthy births. Each mother is tested for infectious diseases and has her medical history screened. Proper consent is received from each family prior to donation.

Before they are approved for use in treatment all umbilical cord-derived stem cells are screened for infectious diseases to International Blood Bank Standards.

Only about 1 in 10 donated umbilical cords pass our rigorous screening process.

Through retrospective analysis of our cases, weve identified proteins and genes that allow us to screen several hundred umbilical cord donations to find the ones that we know are most effective. We only use these cells and we call them golden cells.

We go through a very high throughput screening process to find cells that we know have the best anti-inflammatory activity, the best immune modulating capacity, and the best ability to stimulate regeneration.

The bodys immune system is unable to recognize umbilical cord-derived mesenchmyal stem cells as foreign and therefore they are not rejected. HUCT stem cells have been administered thousands of times at the Stem Cell Institute and there has never been a single instance rejection (graft vs. host disease). Umbilical cord-derived mesenchymal stem cells also proliferate/differentiate more efficiently than older cells, such as those found in the fat and therefore, they are considered to be more potent.

Rob Waddell Defeats Kidney Disease

Rob Waddell knew at an early age that he would need a kidney transplant. His mother has had two transplants and polycystic kidney disease runs in his family. "Ive had two uncles thatve died from this disease. At early ages. I mean they went on dialysis, they had a transplant, something happened, theyre no longer here. Their kids are, without, without a dad!", Rob said.

So when his doctor told him he had to go on dialysis and that a transplant was imminent it was no surprise. Having watched his mother suffer the ups and downs of taking anti-rejection drugs her whole life, he was thrilled to find out there was another option. He entered a clinical trial whereby he would receive an adult stem cell transplant from his kidney donor at the time of the kidney transplant surgery. The donors adult stem cells would allow Rob to accept the same donors kidney, essentially re-training his immune system so that it would recognize the donor kidney as part of Robs own body.

Rob says, "Well, I decided to do the stem cell transplant because I didnt want to live the rest of my life on immune rejection drugs. The good and the bad of immune suppressant drugs is they let the kidney stay in your body. The bad part is that slowly over time it kills the kidney. Its toxic to the kidney. So those drugs, over time, will cause the kidney to fail. My wife, Karen, she, when I proposed the idea of me doing this stem cell study, she was really kind of concerned. I mean she didnt want me to do it because it was new."

Karen Waddell remembers what she said when she heard about the adult stem cell transplant, "I told him I was totally against it from the beginning. Didnt like it. I said, you can just have a normal transplant. Your mom has lived through it. You know, well just adjust."

Rob says, "Seeing my mother go through the repercussions of having kidney disease and the transplant and immune rejection drugs, probably was the number one foundation for me pursuing this."

After the stem cell infusion, Karen says, Rob was like a new man. "Its like hes rejuvenated. Its amazing. He's alert. All his faculties are working great. And for him to be just drug-free, oh its wonderful!

"We call him my fifth child and other people that know us too, theyll tease, because you will see him rip-sticking around the neighborhood, or on the trampoline. So Im thankful that he was able to just be determined and have that drive and the foresight to know that he was going to get those stem cells.

Today Rob lives a full and active life chasing four kids around the soccer, baseball and lacrosse fields of Louisville, Kentucky.

"I feel so fortunate, because Ive been blessed with this. I mean truly a new lease on life. I feel fantastic. My kids could tell you that. I mean I wear them out half the time and I didnt before.

Actually, almost every day since then, I just walk around and Im like, Wow! I feel so goodI mean is this really happening? These adult stem cells to me were a chance to live a normal life.and its amazing."

Cataracts are a clouding of the lenses in your eyes. They affect vision and are very common in older people. More than 22 million Americans have cataracts. They are the leading cause of blindness in the world. By age 80, more than half of all people in the United States either will have a cataract or have had cataract surgery.

Common symptoms are:

Treatment with new glasses, brighter lighting, anti-glare sunglasses, or magnifying lenses can help at first. Surgery is also an option. It involves removing the cloudy lens and replacing it with an artificial lens. Wearing sunglasses and a hat with a brim to block ultraviolet sunlight may help to delay cataracts.

Glaucoma damages the eye's optic nerve and is a leading cause of blindness. It usually happens when the fluid pressure inside the eyes slowly rises, damaging the optic nerve. Often there are no symptoms at first, but a comprehensive eye exam can detect it. About 2.3 million Americans suffer from glaucoma. It is estimated that an additional 2 million have the disease but don't know it.

Glaucoma is a leading cause of blindness among African Americans and Hispanics. African Americans experience glaucoma at a rate three times that of whites. They suffer blindness four times more frequently. Between the ages of 45 and 64, glaucoma is 15 times more likely to cause blindness in African Americans than in whites.

People at risk should get eye exams at least every two years, especially:

Treatment usually includes prescription eye drops and/or surgery. There is no "cure" for glaucoma. Early diagnosis and treatment can control glaucoma before vision loss or blindness occurs. New research is focusing not only on lowering pressure inside the eye, but also exploring medications to protect and preserve the optic nerve from the damage that causes vision loss. There has been progress in understanding the genetics of glaucoma in the last few years, including the discovery of genes found to be associated with many forms of glaucoma.

Macular degeneration, often called age-related macular degeneration (AMD), is a leading cause of vision loss in Americans 60 and older. It is a disease that destroys the sharp, central vision needed to see objects clearly. This affects the ability to read, drive, watch television, and do routine daily tasks.

AMD affects the macula, the part of the eye that allows you to see fine detail. It does not hurt, but causes cells in the macula to die. In some cases, AMD advances so slowly that people notice little change in vision. In others, the disease progresses faster and may lead to vision loss in both eyes. There are two kinds of AMDwet and dry. Wet AMD occurs when abnormal blood vessels behind the retina start to grow under the macula. Dry AMD occurs when the light-sensitive cells in the macula slowly break down, gradually blurring central vision in the affected eye.

Treatment: Although there is no cure for AMD at this time, regular eye exams can detect the disease so treatment can be most effective. Findings from the NEI-supported Age-Related Eye Disease Study (AREDS) show that a specific combination of vitamins and minerals can help slow the progression of advanced AMD. Treatments for wet AMD include laser surgery, photodynamic therapy, and drugs injected into the eye. The NEI recently started a clinical trial to compare the relative safety and effectiveness of two drugs currently used to treat wet AMD. The two drugs, which are injected into the eye, are Lucentis (ranibizumab) and Avastin (bevacizumab). NEI is also supporting AREDS2 to learn whether a modified combination of vitamins and minerals can further help people with AMD.

Diabetic retinopathy is caused by diabetes. It affects the retina, the light-sensitive tissue at the back of the eye, and causes the most blindness in U.S. adults. It affects the vision of more than half of the 18 million people diagnosed with diabetes age 18 or older.

People with diabetes should have a complete eye exam through dilated pupils at least once a year.

Treatment: Diabetic retinopathy is treated with surgery or laser surgery. With timely treatment; adequate control of blood sugar, blood pressure, and cholesterol levels; and regular follow up, 90 percent of all cases of blindness from diabetes can be prevented.

Breast cancer has a higher incidence in obese women …

An international team of researchers, with the participation of the University of Granada (UGR), has revealed new data on why breast cancer has a higher incidence and is more aggressive in obese people. The reason is that peritumoral fat (the fat around the tumor) facilitates the expansion and invasion of cancer stem cells (CSCs), which are responsible of the onset and growth of the tumor.

CSCs are found in tumors in a very small proportion, and their main characteristic is that they are responsible of metastasis originating in parts of the body far from the original tumor. Conventional chemotherapy and radiotherapy treatments are not capable of eliminating said CSCs, and for that, it's very common that, after the first response to the treatment, many cancer patients suffer a relapse.

This new research work has been lead by the University of Miami (Florida, United States), and it has counted with the participation of researchers from Granada's University Hospital Center and from the UGR "Terapias avanzadas: diferenciacin, regeneracin y cncer" (Advanced therapies: differentiation, regeneration and cancer) research team. Both groups of researchers belong to the Biosanitary Institute of Granada (ibs. GRANADA).

Mechanisms yet to be clarified

The consequences of the obesity epidemic on cancer morbidity and mortality are very serious. In fact, it is estimated that, nowadays, up to 20% of cancer-related deaths may be attributable to obesity.

Obese women have a greater risk of suffering breast cancer after menopause, and they have a worse progression of the disease no matter their age, but the mechanisms by which obesity contributes to the development and progression of cancer aren't clear yet. Obesity-related fat causes local inflammation and prevents adipocytes (the cells forming said fat) from maturing.

For this research, carried out in mice and published in the journal Cancer Research, researchers assessed the effects of coculturing adipocytes with breast cancer cells on tumor aggressivity, capacity of local invasion and metastatic potential of said tumor.

The results show that the interaction between tumor cells and immature adipocytes near the tumor during the first stages of breast cancer increased the secretion of cytokines (proinflammatory proteins).

"Said cytokines cause a greater expansion of highly metastatic CSCs," UGR professor Juan Antonio Marchal Corrales, one of the authors of this paper, explains.

Preclinical rationale

In addition, the researchers have described the mechanism by which this process takes place and its relation with the activation of the SRC Kinase protein. In turn, said protein induces the activation of the Sox2 transcription factor (essential to maintain stem cells characteristics) and of a small RNA molecule called miRNA-302b.

"The prolonged coculture of tumor cells with immature adipocytes or cytokines increased the proportion of CSCs (which had the ability to form new tumors), the presence of tumor cells in blood, and the metastatic potential after its implementation in mice -Marchal says-. And last, we found that SRC-Kinase-inhibiting drugs decrease the production of cytokines and CSCs."

These findings reveal new insights underlying increased breast cancer mortality in obese individuals and provide a novel preclinical rationale to test the efficacy of SRC inhibitors for breast cancer treatment.

Exercise boosts health by influencing stem cells to become …

McMaster researchers have found one more reason to exercise: working out triggers influential stem cells to become bone instead of fat, improving overall health by boosting the body's capacity to make blood.

The body's mesenchymal stem cells are most likely to become fat or bone, depending on which path they follow.

Using treadmill-conditioned mice, a team led by the Department of Kinesiology's Gianni Parise has shown that aerobic exercise triggers those cells to become bone more often than fat.

The exercising mice ran less than an hour, three times a week, enough time to have a significant impact on their blood production, says Parise, an associate professor.

In sedentary mice, the same stem cells were more likely to become fat, impairing blood production in the marrow cavities of bones.

The research appears in a new paper published by the Journal of the Federation of American Societies for Experimental Biology.

"The interesting thing was that a modest exercise program was able to significantly increase blood cells in the marrow and in circulation," says Parise. "What we're suggesting is that exercise is a potent stimulus -- enough of a stimulus to actually trigger a switch in these mesenchymal stem cells."

The composition of cells in the bone marrow cavity has an important influence on the productivity of blood stem cells.

In ideal conditions, blood stem cells create healthy blood that boosts the immune system, permits the efficient uptake of oxygen, and improves the ability to clot wounds.

Bone cells improve the climate for blood stem cells to make blood.

But when fat cells start to fill the bone marrow cavity -- a common symptom of sedentary behavior -- blood stem cells become less productive, and conditions such as anemia can result.

The findings add to the growing list of established benefits of exercise, Parise says, and suggest that novel non-medicinal treatments for blood-related disorders may be in the future.

"Some of the impact of exercise is comparable to what we see with pharmaceutical intervention," he says. "Exercise has the ability to impact stem cell biology. It has the ability to influence how they differentiate."

Cell therapy originated in the nineteenth century when scientists experimented by injecting animal material in an attempt to prevent and treat illness.[2] Although such attempts produced no positive benefit, further research found in the mid twentieth century that human cells could be used to help prevent the human body rejecting transplanted organs, leading in time to successful bone marrow transplantation.[3]

Today two distinct categories of cell therapy are recognized.[1]

The first category is cell therapy in mainstream medicine. This is the subject of intense research and the basis of potential therapeutic benefit.[4] Such research can be controversial when it involves human embryonic material.

The second category is in alternative medicine, and perpetuates the practice of injecting animal materials in an attempt to cure disease. This practice, according to the American Cancer Society, is not backed by any medical evidence of effectiveness, and can have deadly consequences.[1]

Cell therapy can be defined as therapy in which cellular material is injected into a patient.[1]

There are two branches of cell therapy: one is legitimate and established, whereby human cells are transplanted from a donor to a patient; the other is dangerous alternative medicine, whereby injected animal cells are used to attempt to treat illness.[1]

The origins of cell therapy can perhaps be traced to the nineteenth century, when Charles-douard Brown-Squard (18171894) injected animal testicle extracts in an attempt to stop the effects of aging.[2] In 1931 Paul Niehans (18821971) who has been called the inventor of cell therapy attempted to cure a patient by injecting material from calf embryos.[1] Niehans claimed to have treated many people for cancer using this technique, though his claims have never been validated by research.[1]

In 1953 researchers found that laboratory animals could be helped not to reject organ transplants by pre-inoculating them with cells from donor animals; in 1968, in Minnesota, the first successful human bone marrow transplantation took place.[3]

Bone marrow transplants have been found to be effective, along with some other kinds of human cell therapy for example in treating damaged knee cartilage.[1] In recent times, cell therapy using human material has been recognized as an important field in the treatment of human disease.[4] The experimental field of Stem cell therapy has shown promise for new types of treatment.[1]

In mainstream medicine, cell therapy is supported by a distinct healthcare industry which sees strong prospects for future growth.[5][6]

In allogeneic cell therapy the donor is a different person to the recipient of the cells.[7] In pharmaceutical manufacturing, the allogenic methodology is promising because unmatched allogenic therapies can form the basis of "off the shelf" products.[8] There is research interest in attempting to develop such products to treat conditions including Crohn's disease[9] and a variety of vascular conditions.[10]

Research into human embryonic stem cells is controversial, and regulation varies from country to country, with some countries banning it outright. Nevertheless, these cells are being investigated as the basis for a number of therapeutic applications, including possible treatments for diabetes[11] and Parkinson's disease.[12]

Cell therapy is targeted at many clinical indications in multiple organs and by several modes of cell delivery. Accordingly, the specific mechanisms of action involved in the therapies are wide ranging. However, there are two main principles by which cells facilitate therapeutic action:

Neural stem cells (NSCs) are the subject of ongoing research for possible therapeutic applications, for example for treating a number of neurological disorders such as Parkinson's disease and Huntington's disease.[20]

MSCs are immunomodulatory, multipotent and fast proliferating and these unique capabilities mean they can be used for a wide range of treatments including immune-modulatory therapy, bone and cartilage regeneration, myocardium regeneration and the treatment of Hurler syndrome, a skeletal and neurological disorder.[21]

Researchers have demonstrated the use of MSCs for the treatment of osteogenesis imperfecta (OI). Horwitz et al. transplanted bone marrow (BM) cells from human leukocyte antigen (HLA)-identical siblings to patients suffering from OI. Results show that MSCs can develop into normal osteoblasts, leading to fast bone development and reduced fracture frequencies.[22] A more recent clinical trial showed that allogeneic fetal MSCs transplanted in utero in patients with severe OI can engraft and differentiate into bone in a human fetus.[23]

Besides bone and cartilage regeneration, cardiomyocyte regeneration with autologous BM MSCs has also been reported recently. Introduction of BM MSCs following myocardial infarction (MI) resulted in significant reduction of damaged regions and improvement in heart function. Clinical trials for treatment of acute MI with Prochymal by Osiris Therapeutics are underway. Also, a clinical trial revealed huge improvements in nerve conduction velocities in Hurlers Syndrome patients infused with BM MSCs from HLA-identical siblings.[24]

HSCs possess the ability to self-renew and differentiate into all types of blood cells, especially those involved in the human immune system. Thus, they can be used to treat blood and immune disorders. Since human bone marrow (BM) grafting was first published in 1957,[25] there have been significant advancements in HSCs therapy. Following that, syngeneic marrow infusion[26] and allogeneic marrow grafting[27] were performed successfully. HSCs therapy can also render its cure by reconstituting damaged blood-forming cells and restoring the immune system after high-dose chemotherapy to eliminate disease.[28]

There are three types of HSCT: syngeneic, autologous, and allogeneic transplants.[21] Syngeneic transplantations occur between identical twins. Autologous transplantations use the HSCs obtained directly from the patient and hence do not cause any complications of tissue incompatibility; whereas allogeneic transplantations involve the use of donor HSCs, either genetically related or unrelated to the recipient. To lower the risks of transplant, which include graft rejection and Graft-Versus-Host Disease (GVHD), allogeneic HSCT must satisfy compatibility at the HLA loci (i.e. genetic matching to reduce the immunogenicity of the transplant). Mismatch of HLA loci would result in treatment-related mortality and higher risk of acute GVHD.[29]

In addition to BM derived HSCs, the use of alternative sources such as umbilical cord blood (UCB) and peripheral blood stem cells (PBSCs) has been increasing. In comparison with BM derived HSCs recipients, PBSCs recipients afflicted with myeloid malignancies reported a faster engraftment and better overall survival.[30] However, this was at the expense of increased rate of GVHD.[31] Also, the use of UCB requires less stringent HLA loci matching, although the time of engraftment is longer and graft failure rate is higher.[32][33]

In alternative medicine, cell therapy is defined as the injection of non-human cellular animal material in an attempt to treat illness.[1]Quackwatch labels this as "senseless", since "cells from the organs of one species cannot replace the cells from the organs of other species" and because a number of serious adverse effects have been reported.[34]

Of this alternative, animal-based form of cell therapy, the American Cancer Society say: "Available scientific evidence does not support claims that cell therapy is effective in treating cancer or any other disease. In may in fact be lethal ...".[1]
