2019: the year gene therapy came of age – RFI

In the summer, a mother in Nashville with a seemingly incurable genetic disorder finally found an end to her suffering -- by editing her genome.

Victoria Gray's recovery from sickle cell disease, which had caused her painful seizures, came in a year of breakthroughs in one of the hottest areas of medical research -- gene therapy.

"I have hoped for a cure since I was about 11," the 34-year-old told AFP in an email.

"Since I received the new cells, I have been able to enjoy more time with my family without worrying about pain or an out-of-the-blue emergency."

Over several weeks, Gray's blood was drawn so doctors could get to the cause of her illness -- stem cells from her bone marrow that were making deformed red blood cells.

The stem cells were sent to a Scottish laboratory, where their DNA was modified using Crispr/Cas9 -- pronounced "Crisper" -- a new tool informally known as molecular "scissors."

The genetically edited cells were transfused back into Gray's veins and bone marrow. A month later, she was producing normal blood cells.

Medics warn that caution is necessary but, theoretically, she has been cured.

"This is one patient. This is early results. We need to see how it works out in other patients," said her doctor, Haydar Frangoul, at the Sarah Cannon Research Institute in Nashville.

"But these results are really exciting."

In Germany, a 19-year-old woman was treated with a similar method for a different blood disease, beta thalassemia. She had previously needed 16 blood transfusions per year.

Nine months later, she is completely free of that burden.

For decades, the DNA of living organisms such as corn and salmon has been modified.

But Crispr, invented in 2012, made gene editing more widely accessible. It is much simpler than preceding technology, cheaper and easy to use in small labs.

The technique has given new impetus to the perennial debate over the wisdom of humanity manipulating life itself.

"It's all developing very quickly," said French geneticist Emmanuelle Charpentier, one of Crispr's inventors and the cofounder of Crispr Therapeutics, the biotech company conducting the clinical trials involving Gray and the German patient.

- Cures -

Crispr is the latest breakthrough in a year of great strides in gene therapy, a medical adventure started three decades ago, when the first TV telethons were raising money for children with muscular dystrophy.

Scientists practising the technique insert a normal gene into cells containing a defective gene.

It does the work the original could not -- such as making normal red blood cells, in Victoria's case, or making tumor-killing super white blood cells for a cancer patient.

Crispr goes even further: instead of adding a gene, the tool edits the genome itself.

After decades of research and clinical trials on a genetic fix to genetic disorders, 2019 saw a historic milestone: approval to bring to market the first gene therapies for a neuromuscular disease in the US and a blood disease in the European Union.

They join several other gene therapies -- bringing the total to eight -- approved in recent years to treat certain cancers and an inherited blindness.

Serge Braun, the scientific director of the French Muscular Dystrophy Association, sees 2019 as a turning point that will lead to a medical revolution.

"Twenty-five, 30 years, that's the time it had to take," he told AFP from Paris.

"It took a generation for gene therapy to become a reality. Now, it's only going to go faster."

Just outside Washington, at the National Institutes of Health (NIH), researchers are also celebrating a "breakthrough period."

"We have hit an inflection point," said Carrie Wolinetz, NIH's associate director for science policy.

These therapies are exorbitantly expensive, however, costing up to $2 million -- meaning patients face grueling negotiations with their insurance companies.

They also involve a complex regimen of procedures that are only available in wealthy countries.

Gray spent months in hospital getting blood drawn, undergoing chemotherapy, having edited stem cells reintroduced via transfusion -- and fighting a general infection.

"You cannot do this in a community hospital close to home," said her doctor.

However, the number of approved gene therapies will increase to about 40 by 2022, according to MIT researchers.

They will mostly target cancers and diseases that affect muscles, the eyes and the nervous system.

- Bioterrorism -

Another problem with Crispr is that its relative simplicity has triggered the imaginations of rogue practitioners who don't necessarily share the medical ethics of Western medicine.

Last year in China, scientist He Jiankui triggered an international scandal -- and his excommunication from the scientific community -- when he used Crispr to create what he called the first gene-edited humans.

The biophysicist said he had altered the DNA of human embryos that became twin girls Lulu and Nana.

His goal was to create a mutation that would prevent the girls from contracting HIV, even though there was no specific reason to put them through the process.

"That technology is not safe," said Kiran Musunuru, a genetics professor at the University of Pennsylvania, explaining that the Crispr "scissors" often cut next to the targeted gene, causing unexpected mutations.

"It's very easy to do if you don't care about the consequences," Musunuru added.

Despite the ethical pitfalls, restraint seems mainly to have prevailed so far.

The community is keeping a close eye on Russia, where biologist Denis Rebrikov has said he wants to use Crispr to help deaf parents have children without the disability.

There is also the temptation to genetically edit entire animal species -- malaria-causing mosquitoes in Burkina Faso or mice hosting ticks that carry Lyme disease in the US.

The researchers in charge of those projects are advancing carefully, however, fully aware of the unpredictability of chain reactions on the ecosystem.

Charpentier doesn't believe in the more dystopian scenarios predicted for gene therapy, including American "biohackers" injecting themselves with Crispr technology bought online.

"Not everyone is a biologist or scientist," she said.

And the possibility of military hijacking to create soldier-killing viruses or bacteria that would ravage enemies' crops?

Charpentier thinks that technology generally tends to be used for the better.

"I'm a bacteriologist -- we've been talking about bioterrorism for years," she said. "Nothing has ever happened."

Merck in $1.3B showdown with insurers over 2017 ransomware attack Bloomberg – Endpoints News

When Merck was hit with a ransomware attack in 2017, the pharma giant was in fact just collateral damage from a virus Russia aimed at Ukraine, and the company is struggling to recoup its losses because its $1.75 billion insurance plan doesnt cover acts of war.

Those are details from a richly reported Bloomberg Newsstory out yesterday exploring how a geopolitical fight in Eastern Europe accidentally entangled a New Jersey-based pharmaceutical company and sparked lawsuits with major ramifications for the future of insurance and cybercrime.

Merck employees logging on to their computers on June 27, 2017 were greeted with a polite message in pink letters: Ooops, your important files are encrypted. We guarantee that you can recover all your files safely and easily. All you need to do is submit the payment. The payment was $300 in bitcoin, per computer.

Early on, it was clear that Merck was one victim of a global attack that also hit Danish shipping company Maersk, American food company Mondelez, French construction giant Saint-Gobain and even the systems monitoring the Chernobyl nuclear power stations, among others.

Unlike Chernobyl, though, it appears that Merck was not an intended target. The attack was dubbed NotPetya, a creation of the GRU Russian military intelligence agency (the same one that attacked the Democratic National Committee), and it was designed to strike companies and agencies in Ukraine, a country that had been in conflict with Russia since 2014. But, per Bloomberg, NotPetya contaminated a tax software application, M.E.Doc, that was running on a server in Mercks Ukraine office.

From there, it spread to the pharma giants headquarters, where it would eliminate in some cases years of research, cripple Gardisal 9 production facilities and eventually cause (by Mercks estimate) $1.3 billion in damages. Merck, though, had a property insurance plan worth up to $1.75 billion that covered computer data, coding and software (after a $150 million deductible). But when Merck went to activate the plan, most of their 30 insurers rejected them. Your plan doesnt cover damages from military action, they told shocked Merck officials.

What followed were, not surprisingly,a string of lawsuits, with Merck claiming that it was hit by a cyber not a military event. These lawsuits, Bloomberg reports, are being watched for the precedents they may set around how future cybercrime is classified.

The insurers are trying to prove two things: that the attack really did come from Russia and that Merck was not as vigilant as it could have been in protecting their data. Merck, as Endpoints News reportedshortly after the attack, had missed two opportunities to inoculate themselves against the virus before they were struck.

On Russia, the insurers have gotten a hand from the White House. Last year, the Trump Administration wrote without equivocation that the attack was part of the Kremlins ongoing effort to destabilize Ukraine and demonstrates ever more clearly Russias involvement in the ongoing conflict.

When the president of the United States comes out and says, Its Russia, its going to be hard to fight, Jake Williams, a former National Security Agency hacker who now helps companies hunt for vulnerabilities in their computer networks, told Bloomberg. Ill be surprised if the insurance companies dont get a win. This is as solid a case as theyre going to get.

But some legal experts expressed greater skepticism of the insurers case. All signs may point to Russian culpability but when it comes to cyber, its not clear what military action means.

Its not going to be an easy case for a judge in the U.S. to declare that this was an act of war, Catherine Lotrionte, a former CIA lawyer whos taught at Georgetown University, told Bloomberg.

Former Google CEO Eric Schmidt has a vision for healthcare.

He explained it on a recent episode of the podcast Theory and Practice, which focuses on the intersection of biology and computer science. The podcast is produced by Google AI and the venture arm GV, which was formerly known as Google Ventures.

Schmidt, a billionaire who studied electrical engineering at Princeton University, was Googles CEO for a decade and later served as executive chairman. He left that role at Googles parent company, Alphabet, in 2018.

Schmidt has long held beliefs about the important role of big data and data sharing, he said. Data sharing has faced obstacles in healthcare, where the privacy of personal health data is a big concern.

Schmidt said he believed that better data access and sharing would improve healthcare.

If you look at the medical system in America, it was never designed in a way that you and I would sort of agree as rational. The incentives are misaligned; the databases are poor, Schmidt told the cohosts Anthony Philippakis, a venture partner at GV, and Alex Wiltschko, a senior research scientist with Google AI.

I believe that because of the gains in machine learning and data analysis, we have an opportunity to rethink some of those underlying assumptions, he added.

Because of the disjointed nature of the US healthcare system, Schmidt said that all medical data should be in one place that is easy for the doctor and patient to access.

When I go to the doctor, I want to give them a login and a password for me, and when they log in, I want them to see all of my medical data from everywhere, Schmidt said.

As Schmidt said in the podcast episode that, oftentimes, when a patient has to have multiple tests done by different specialists, the medical information is not shared between their systems. This is just one example of the disconnected communications that Schmidt says could easily be fixed.

Another important point to fix is making large amounts of health data available for research purposes. Schmidt said the privacy issue could be resolved by allowing patients to opt out of data collection. Otherwise, that data is, by custom, made available for research purposes to make the system stronger, he said.

Schmidt said that a key step would be to put all medical data on the cloud, where it can more easily be accessed and analyzed.

Right now, the majority of medical data is not even in the electronic-health-record system, its in other systems that are sitting around in the hospital, Schmidt said. But the work is underway, and all EHRs will be cloud-based quite soon.

For Schmidt, cloud computing is beneficial in healthcare because its often less expensive and can support the massive amounts of data the industry constantly produces.

With cloud computing, you know the system wont fall over, he said.

But Schmidt doesnt just want to store electronic health records on the cloud. He also wants to collect other clinical data from hospital systems and eventually all clinical data in the healthcare industry.

Wed have a much fuller picture of whats going on in a medical care setting, and that would allow us to do better data analytics, better prediction and better healthcare, Schmidt said.

Schmidt said he believed healthcare systems would be quick to move to the cloud if it could prove to be beneficial for the patient and doctor by saving time and peoples lives.

With access to more patient data, Schmidt believes computers can help the healthcare system by providing more accurate medical diagnoses for a larger group of people.

Big data can provide better predictive analytics, he said. He said deep data science could be used to help doctors make better decisions.

I want a computer to be able to say, Heres your history; heres what we think is going on, and give advice using deep data science, doing deep predictive analytics, and AI in general to predict what the doctor should do next, Schmidt said. I think this will lead to a revolution in healthcare in terms of productivity and, most important, my health and your health and everyones.

Stem cell therapy approved for MS patients in Scotland – HeraldScotland

MULTIPLE sclerosis campaigners have hailed a huge step forward for patients in Scotland after a stem cell therapy was recommended for use on the NHS for the first time.

Haematopoietic stem cell transplantation (HSCT) has been described as a game-changer for MS after an international clinical trial showed that it could reboot patients immune systems and halt the progress of the disease.

Some patients who had been in wheelchairs prior to treatment said their condition improved so dramatically it was like they had never been diagnosed with MS.

The Scottish Health Technologies Group (SHTG) said there is now sufficient evidence for it to recommend making HSCT available on the NHS in Scotland to MS patients who have the relapsing-remitting form of the disease, and who were not responding to drug treatments.

Iain Robertson, chairman of the SHTG, said: Our committee members were able to advise that this treatment should be considered for those with this particular type of MS who have not responded to treatment with disease-modifying therapies.

We hope that our advice will be of use in helping decide the best course of treatment for these patients.

The SHTG also stressed that patients must be made aware of the demands, risks and uncertainties of the treatment, which uses chemotherapy to wipe out patients' 'faulty' immune systems before replenishing it with a transplant of stem cells harvested from their own bone marrow.

It puts patients at high risk from infections, which can be fatal, but the theory is that the treatment works by enabling patients to 'reset' their immune system to stop it attacking the central nervous system as is the case in MS.

HSCT is not considered an effective treatment for patients with the progressive form of MS, however, as stem cells cannot regrow nerves or repair damaged myelin - the protective sheath which coats nerves.

It will also be unavailable to patients with relapsing-remitting MS who no longer show signs of inflammation on an MRI brain scan.

Scotland has one of the highest rates of MS in the world, but until now Scottish patients seeking HSCT have had to travel overseas to Mexico, Russia and Israel and bankroll their own private treatment at a cost of around 40-60,000.

It has also been available privately in London since 2017, but with a 100,000 price tag.

A small number of MS patients in England have been able to access the treatment on the NHS, however, because there are clinical trials into HSCT taking place at NHS hospitals in Sheffield and London.

Morna Simpkins, director of MS Society Scotland, said: The decision from SHTG to approve HSCT for the treatment of MS is good news and could help in the development of a clear pathway, for people who could potentially benefit, to access it.

We will push to ensure that this decision leads to real change for people with MS by continuing to engage with other groups to offer the treatments, including HSCT, which are right for them.

The SHTG said eligible patients must have equal access to the procedures regardless of where they live, but it is unlikely all health boards will be able to provide it.

The MS Society wants a centre, or centres, of excellence set up where patients from across Scotland can be referred.

Lucy Clarke from the Scottish HSCT Network said the recommendation was "a huge step forward" for people in Scotland living with MS.

Ms Clarke underwent HSCT in Russia and credits it with substantially reversing her disability.

She added: This important decision supports HSCT as a treatment option where other treatments have failed. We will continue to push so that this treatment is available to people in Scotland who need it.

A Scottish Government spokeswoman said: We are grateful to the Scottish Health Technologies Group for this important work.

"NHS Boards are expected to consider their advice on technologies in the planning and provision of its services and clinicians are expected to follow their professional judgement, working within the management structure of their Board.

We will work closely with MS Society Scotland, other third sector bodies and the clinical community to consider what the Technologies Groups findings means for provision in Scotland, including the information that needs to be available to people about eligibility and risks.
