Stem cell treatment: Controversial for humans, but not for pets

PHOENIX, Ariz. -

Stem cell research and therapy on humans has traveled a long and often politically troubled path.

Not so for pets, where stem cell treatment has been used for nearly 10 years and now it is so routine, and so successful, it can be done in a day.

Ava is a 90 pound, 2-year-old Akita, who is about to undergo stem cell surgery. A little IV, a little anesthesia and Ava is out.

"It is used for arthritis mostly," said Dr. Velvet Edwards.

Ava is just beginning her day at Pecan Grove Veterinary Hospital in Tempe. Dr. Edwards oversees the stem cell procedure.

"Stem cells are healing cells, so they seek out area of injury damage or destruction," explained Edwards. "They accelerate healing and help the animal, the patient, the pet just use their own natural abilities to get better."

Veterinary stem cells are harvested from the animal's own fat cells. They are separated and processed by machinery right inside the vet's office and then injected back into the dog's trouble spots.

Thanks to new technology developed by Meti Vet, the process is completed in just a day.

"The pet comes in the morning, it's anesthetized and I collect about two to four grams of fat usually behind the shoulder blade," said Edwards. "Then I hand that fat over to my technicians to run it through a series of steps.. basically to dissolve the fat and get down to a little stem cell pellet... Then we take that pellet and we reconstitute it and make it injectable. I will put it back into the animal's body wherever I need it later that day."

Even though stem cell manufacture has just been licensed in this country, the Governments ongoing failure to legislate in this area means pharmaceutical giants will still be wary of investing here, according to an expert in stem cell therapy.

Once the stem cells are harvested from the bone marrow of adult donors, they are grown in the Galway laboratory to generate sufficient quantities.

The first clinical trial using these stem cells is being funded by the Health Research Board and Science Foundation Ireland and will investigate the safety of using mesenchymal stem cells (MSCs) isolated from bone marrow for the treatment of critical limb ischemia, a complication associated with diabetes which can lead to limb amputation.

John ODea of the Irish Medical Devices Association (IMDA) said the centre was a key step.

I look forward to seeing its continued growth to assist in developing the skill sets and techniques that will be needed to embrace the new manufacturing opportunities that this exciting area will bring, he said.

The centre, one of a handful in Europe authorised for stem cell manufacture, has been developed by researchers at NUIGs regenerative medicine institute.

However, Dr Stephen Sullivan, chief scientific officer with the Irish Stem Cell Foundation warned all stem cell research operates at a pan-global level driven by big pharma and international equity firms and these players will only engage with researchers in countries where there is solid stem cell legislation in place. He welcomed the centre as a first step but said if Ireland is to compete at a top international standard, legislation remains necessary.

ELLSWORTH Visitors to the Bellaire pet crisis center With a Little help From My Friends get an official welcome from Moka.

The Labrador retriever was found behind a Bellaire restaurant in 2011 and now serves as the centers mascot.

Peforming her duties has been increasingly difficult for the dog, who suffers from severe arthritis in her hips. So recently the center turned to Ellsworth veterinarian Christian Randall of North Country Veterinary Services, the first in northern Michigan to offer in-clinic adipose stem cell therapy.

The procedure uses a pets own blood and tissue to produce plasma-rich platelets and stem cells that proliferate growth in damaged areas.

Dormant stem cells are separated from adipose -- fat tissue -- and activated with an LED technology that uses three different wave lengths of light. Then the cells are injected directly into the affected area or administered intravenously to help promote regeneration. The result is a decrease in pain and lameness and increased range of motion.

Its using the bodys own repair cells to repair damage, said Trey Smith, director of laboratory services for MediVet America, which developed the technology Randall uses.

The therapy is the first treatment to help heal and slow the progression of osteoarthritis and degenerative joint disease rather than just cope with the symptoms, said Randall, who saw the results while studying at Virginia Equine Imaging and now plans to use it on equine as well as canine and feline patients.

It concentrates, speeds up and amplifies the bodys own healing power, he said.

Stem cell therapy has been around for a while, but in-clinic availability of the technology is new. Only a handful of veterinarians in Ann Arbor and Grand Rapids offer the services, said Randall, who charges $1,800 to treat a dog or cat. Repeat injections are possible with banked plasma-rich platelets and stem cells.

Before the one-day procedure, veterinarians had to send blood and tissue to an outside lab for processing, a more costly three-day procedure that requires an animal's return visit to the vet for injection.

The U.S. Navy, Office of Naval Research, awarded Vet-Stem a contract to engage in a collaborative study of stem cell biology in marine mammals in 2009. From this, the first peer-reviewed article was published showing successful isolation of stem cells from dolphin fat. Several media outlets featured a story on a panther from the Tallahassee Museum who received stem cell therapy by Vet-Stem for arthritis of the elbow in 2011. After the therapy he was able to stand up and scratch on his favorite tree with both front paws.

New tool assists stem cell therapy

A new tool that could help facilitate future stem cell therapy has recently been identified by a UVM professor and his colleagues, according to UVMs College of Medicine.

The development of this tool could potentially help more than 700,000 Americans who suffer a heart attack each year.

Because stem cells have the potential to develop into a variety of cell types in the body, they may offer a renewable source of replacement cells to treat diseases, conditions and disabilities, and even regenerate damaged tissue and organs.

However, the field of regenerative medicine has struggled to successfully graft cells from culture back into injured tissue.

UVM Associate Professor of Medicine Jeffrey Spees, Ph.D., collaborated with the Center for Gene Therapy at Tulane University. His research team recently set out to develop ways to enhance graft success.

Dr. Spees and his team focused on a type of bone marrow-derived progenitor cell or biological cell that forms stromal cells or connective tissue cells.

They found that the medium contained Connective Tissue Growth Factor (CTGF) and the hormone insulin, and together, they have a synergistic effect, Spees said to UVMs College of Medicine.

The group found that the protective ligands resulted in improved graft success, breaking the record for engraftment.

Embryonic stem cell rejection problem fixed, study says

One of the toughest problems facing embryonic stem cell therapy, immune rejection of transplanted cells, may have been solved, according to a UC San Diego-led research team.

The cells can be made invisible to the immune system by genetically engineering them to make two immune-suppressing molecules, according to the study. Researchers tested the approach in mice given a human immune system. Immune functioning in the rest of the animal remained active.

If the approach works in people, patients receiving transplanted tissue or organs made from embryonic stem cells wouldnt have to take harsh immune-suppressing drugs, said study leader Yang Xu, a UC San Diego professor of biology.

Researchers placed genes in the stem cells to produce the two molecules, called CTLA4-lg and PD-L1, naturally made in the body. The mice accepted transplants of heart and skin cells derived from the engineered stem cells. They rejected transplants derived from regular embryonic stem cells.

The study was published online Thursday in the journal Cell Stem Cell. Its findings will have to be confirmed for safety and effectiveness before human trials can be considered, which will take years.

Three scientists given the paper for comment had mixed reactions. While they praised the works scientific prowess, two said genetically engineering the transplanted cells could cause serious side effects that might preclude their use.

The researchers employed a clever strategy to use the immune systems natural regulatory systems, said Mitchell Kronenberg, president of the La Jolla Institute for Allergy & Immunology.

This is an especially promising approach, because it avoids the toxic side effects of the drugs now used to suppress the rejection response, and therefore this is an important step forward in showing the feasibility of using human embryonic stem cells from unrelated donors, Kronenberg said.

More skeptical were Jeanne Loring, a stem cell researcher at The Scripps Research Institute, and Craig M. Walsh, associate director of the Institute for Immunology at UC Irvine.
