Human embryonic stem cells have the capacity to differentiate into a variety of cell types, making them a valuable source of transplantable tissue for the treatment of numerous diseases, such as Parkinson's disease and diabetes.

But theres one major issue: Embryonic stem cells are often rejected by the human immune system.

Now, researchers from the University of California San Diego may have found an effective way to prevent this rejection in humans. Utilizing a novel humanized mouse model, the scientists have revealed a unique combination of immune suppressing molecules that stop the immune system from attacking the injected stem cells without shutting the system down completely.

This discovery could ultimately help resolve some of the major problems currently limiting the use of embryonic stem cells for certain conditions, paving the way for the development of more effective human stem cell therapies.

This is a generic way of immune suppression, so it could potentially be applied not just for stem cells therapies, but for organ transplants as well, Yang Xu, a professor of biology at UC San Diego and lead author of the study, told FoxNews.com. It can be very broad.

Embryonic stem cells are different from the other cells in a patients body, making them allogenic. This means the immune system will recognize them as foreign agents and attack them.

One way of overcoming this rejection problem is to give patients immunosuppressant drugs, which suppress the entire immune system. While short term use of immunosuppressants has been successful for many organ transplants, embryonic stem cell therapies for chronic diseases require long term use of these drugs which can often be very toxic and increase the risk of cancer.

In order for the patient to really use this therapy, they have to decide: Do they want a lifelong use of immunosuppressant drugs, or are they willing to live with the symptoms of their disease, Xu said.

To figure out a way of bypassing this issue, researchers needed a relevant model that could closely mimic the human immune systems response to embryonic stem cell transplantation. To do this, they took immune deficient lab mice and grafted them with human fetal thymus tissues and hematopoietic stem cells derived from the fetal liver.

Essentially, this created a highly specialized mouse model with very robust T cells capable of effectively rejecting foreign embryonic stem cells just like human T cells.

Stem cell transplant problem solved, UCSD-led study says

One of the toughest problems facing embryonic stem cell therapy, immune rejection of transplanted cells, may have been solved, according to a UC San Diego-led research team.

The cells can be made invisible to the immune system by genetically engineering them to make two immune-suppressing chemicals, according to a study performed in mice given a human immune system. Immune functioning in the rest of the animal remains active.

If the approach works in people, patients receiving transplanted tissue or organs made from embryonic stem cells wouldn't have to take harsh immune-suppressing drugs, said Yang Xu, a UCSD professor of biology.

Researchers placed genes in the stem cells to produce the two chemicals, CTLA4-lg and PD-L1, naturally made in the body. The humanized immune systems of the mice accepted transplants of cells engineered to make the chemicals. The researchers transplanted cardiomyocytes and fibroblasts derived from the engineered stem cells. Transplants derived from regular embryonic stem cells were rejected.

The study was published online Thursday in the journal Cell Stem Cell. Its findings will have to be confirmed for safety and effectiveness in more animal studies before human trials can be considered, which will take years. The mouse model itself was "optimized" for the study to more faithfully reflect the human immune system than other immune models, the study said.

Xu said a further study is being considered in monkeys, a large animal model considered to better reflect human biology than mice.

Embryonic stem cells are being tested along with many other kinds of stem cells to replace diseased or destroyed body parts, such as spinal cord segments and insulin-producing beta cells in the pancreas. All of these cells have advantages and drawbacks. Immune rejection, along with a tendency to form tumors, are two big drawbacks to embryonic stem cells.

San Diego-based ViaCyte is preparing to test a therapy with beta cells within a year. The company encapsulates them in a permeable barrier that allows insulin to diffuse out but prevents the immune system from entering. However, that approach won't worth with transplants that must integrate into the body, such as spinal cord tissue. So a way of turning off the immune system just in those cells is an attractive idea.

Four scientists given the paper to read for comment had mixed reactions. While they praised the work's scientific prowess, three said genetically engineering the transplanted cells could produced serious side effects that might preclude use.

FEMA Search and Rescue Canine Receives Stem Cell Therapy So He Can Continue to Save Lives

Phizer is a seven year old black lab belonging to Ohio Task Force 1 that recently had stem cell therapy by Vet-Stem, Inc. Phizer is trained to cover obstacles and treacherous terrain, climb metal ladders and investigate acres of terrain quickly and efficiently. These skills came in handy when Phizer was assigned to a mission recovering victims from hurricane Sandy.

Handlers Maureen May and Deana Hudgins noticed an intermittent limp in Phizers right rear leg when he first started moving, but got better with exercise. Although the limp was not preventing Phizer from his job, he was started on pain medicine, joint supplements and taken for exams to the local veterinarian. His radiology report showed signs consistent with mild degenerative joint disease in addition to another injury. Deana and Dr. Bailey started Phizer on injectable treatments, laser therapy, and discussed stem cells.

Since Phizers stem cell therapy used his own stem cells, a small portion of fat was collected and sent to Vet-Stems lab in California. Within 48 hrs the doses of stem cells were ready for injection by Dr. Bailey. Dr. Aidan R. Raney performs a checkup on heart attack patient Mark Athens, 52, on Tuesday, Dec. 17, at Scripps Green Hospital in La Jolla. Athens received a stem cell treatment to help his heart recover as part of a clinical trial to determine the treatments safety and effectiveness.

A new stem cell treatment may help heart attack patients do something once thought medically impossible regenerate dead heart muscle.

Scripps Health in La Jolla is one of three centers testing the therapy from Capricor, a Los Angeles biotech company. The cardiac stem cells are meant to boost the hearts natural ability to perform minor repairs. If it works, scars should shrink and functional heart muscle should grow.

Capricor gets the cells from donor hearts, grows them into the amount needed for treatment, then sends them to doctors taking part in what is called the Allstar trial. Doctors inject the cells into the coronary artery, where they are expected to migrate to the heart and encourage muscle regrowth.

The trial has successfully completed Phase 1, which mainly evaluates safety. On Dec. 17, Capricor said it had received permission to begin Phase 2, which will examine efficacy in about 300 patients who will get the treatment or a placebo. The Allstar trial is funded with a $19.7 million disease team grant from the California Institute for Regenerative Medicine, or CIRM, the states stem cell agency.

This is a highly significant announcement for us at CIRM as its the first time weve funded a therapy into a Phase 2 clinical trial, Chairman Jonathan Thomas said in a Dec. 23 statement.

About 600,000 Americans die of heart disease annually, making it the leading cause of death, according to the Centers for Disease Control and Prevention in Atlanta. Even those surviving may be left permanently impaired, if the heart is severely damaged. These are the patients Capricor seeks to help.

Mark Athens received Capricors treatment on Sept. 25, about a month after having a moderate heart attack. The Encinitas resident was the last treated under Phase 1, said Scripps cardiologist Richard Schatz, who performed the procedure. It will take about six months to know whether the treatment worked, Schatz said.

Unlike many trials, Phase 1 was not placebo-controlled, so Athens knows he got the therapy. He appeared cheerful, smiling and bantering with his examining doctor during a Dec. 17 checkup at Scripps Green Hospital.

Stem cell-based gene therapy holds promise for the treatment of devastating genetic skin diseases, but the long-term clinical outcomes of this approach have been unclear. In a study online December 26th in the ISSCR's journal Stem Cell Reports, published by Cell Press, researchers evaluated a patient with a genetic skin disorder known as epidermolysis bullosa (EB) nearly seven years after he had undergone a gene therapy procedure as part of a clinical trial. The study revealed that a small number of skin stem cells transplanted into the patient's legs were sufficient to restore normal skin function, without causing any adverse side effects.

"These findings pave the way for the future safe use of epidermal stem cells for combined cell and gene therapy of epidermolysis bullosa and other genetic skin diseases," says senior study author Michele De Luca of the University of Modena and Reggio Emilia.

EB is a painful condition that causes the skin to be very fragile and to blister easily, and it can also cause life-threatening infections. Because there is no cure for the disease, current treatment strategies focus on relieving symptoms. To evaluate stem cell-based gene therapy as a potential treatment, De Luca and his colleagues previously launched a phase I/II clinical trial at the University of Modena and recruited an EB patient named Claudio. The researchers took skin stem cells from Claudio's palm, corrected the genetic defect in these cells, and then transplanted them into Claudio's upper legs.

In the new study, De Luca and his team found that this treatment resulted in long-term restoration of normal skin function. Nearly seven years later, Claudio's upper legs looked normal and did not show signs of blisters, and there was no evidence of tumor development. Remarkably, a small number of transplanted stem cells was sufficient for long-lasting skin regeneration.

Even though Claudio's skin had undergone about 80 cycles of renewal during this time period, the transplanted stem cells still retained molecular features of palm skin cells and did not adopt features of leg skin cells. "This finding suggests that adult stem cells primarily regenerate the tissue in which they normally reside, with little plasticity to regenerate other tissues," De Luca says. "This calls into question the supposed plasticity of adult stem cells and highlights the need to carefully chose the right type of stem cell for therapeutic tissue regeneration."

Dec. 17, 2013 A systematic survey of the scientific literature shows that stem cell therapy can have a statistically significant impact on animal models of spinal cord injury, and points the way for future studies.

Spinal cord injuries are mostly caused by trauma, often incurred in road traffic or sporting incidents, often with devastating and irreversible consequences, and unfortunately having a relatively high prevalence (250,000 patients in the USA; 80% of cases are male). This question is addressed in a paper published 17th December in the open access journal PLOS Biology by Ana Antonic, David Howells and colleagues from the Florey Institute and the University of Melbourne, Australia, and Malcolm MacLeod and colleagues from the University of Edinburgh, UK.

Stem cell therapy aims to use special regenerative cells (stem cells) to repopulate areas of damage that result from spinal cord injuries, with the hope of improving the ability to move ("motor outcomes") and to feel ("sensory outcomes") beyond the site of the injury. Many studies have been performed that involve animal models of spinal cord injury (mostly rats and mice), but these are limited in scale by financial, practical and ethical considerations. These limitations hamper each individual study's statistical power to detect the true effects of the stem cell implantation.

This new study gets round this problem by conducting a "meta-analysis" -- a sophisticated and systematic cumulative statistical reappraisal of many previous laboratory experiments. In this case the authors assessed 156 published studies that examined the effects of stem cell treatment for experimental spinal injury in a total of about 6000 animals.

Overall, they found that stem cell treatment results in an average improvement of about 25% over the post-injury performance in both sensory and motor outcomes, though the results can vary widely between animals. For sensory outcomes the degree of improvement tended to increase with the number of cells introduced -- scientists are often reassured by this sort of "dose response," as it suggests a real underlying biologically plausible effect.

The authors went on to use their analysis to explore the effects of bias (whether the experimenters knew which animals were treated and which untreated), the way that the stem cells were cultured, the way that the spinal injury was generated, and the way that outcomes were measured. In each case, important lessons were learned that should help inform and refine the design of future animal studies. The meta-analysis also revealed some surprises that should provoke further investigation -- there was little evidence of any beneficial sensory effects in female animals, for example, and it didn't seem to matter whether immunosuppressive drugs were administered or not.

The authors conclude: "Extensive recent preclinical literature suggests that stem cell-based therapies may offer promise; however the impact of compromised internal validity and publication bias means that efficacy is likely to be somewhat lower than reported here."

Scientists from UCLA are now bringing their groundbreaking stem cell science directly to patients in two exciting new clinical trials scheduled to begin in early 2014, thanks to funding from California's stem cell agency.

The new grants to researchers at UCLA's Eli and Edythe Broad Center of Regenerative Medicine and Stem Cell Research, which total nearly $21 million, were announced Dec. 12 at a meeting of the California Institute of Regenerative Medicine (CIRM) Citizen's Oversight Committee. They are apart of the state agency's Disease Team Therapy Development III initiative.

A team led by UCLA's Dr. Dennis Slamon and Dr. Zev Wainberg was awarded nearly $7 million for a clinical trial that will test a new drug targeting cancer stem cells, and UCLA's Dr. Donald Kohn received almost $14 million for a clinical trial that will focus on stem-cell gene therapy for sickle cell disease.

"The CIRM support demonstrates that our multidisciplinary center is at the forefront of translating basic scientific research into new drug and cellular therapies that will revolutionize medicine," said Dr. Owen Witte, director of the UCLA Broad Stem Cell Research Center.

Dennis Slamon and Zev Wainberg: Targeting solid tumor stem cells

This clinical trial builds on Slamon's previous work, partially funded by CIRM, with Wainberg and Dr. Tak Mak, director of the Campbell Family Institute at the University Health Network in Toronto, aimed at developing a drug that targets those stem cells thought to initiate solid cancer tumors.

The AmericanCanadian collaborative team will lead this first in-human Phase 1 trial testing their new therapy, which has received investigational new-drug approval from the U.S. Food and Drug Administration and Health Canada, Canada's therapeutic regulatory agency. The project has been approved to begin enrolling patients in both the U.S. and Canada.
